If you are a member of a Group Health Systems (GHS) Health and Welfare Trust
Client, we have, for your convenience, developed this site to assist you in finding the
closest Preferred Provider (PPO) facility to your physical location.
Remember, utilizing these preferred providers will benefit you, your Health and
Welfare Trust, and most importantly, your health care dollar. Protecting your health
care dollar will mean protecting your quality of health care and your level of Benefits.
GHS has an extensive clinical Laboratory PPO Network in place to better service your
needs. Laboratory tests performed at one of the GHS participating Laboratories will be
paid by the Health and Welfare Plan at deep discounted prices. Laboratory Tests are
performed at no cost to you! If your benefit plan does have co-pay provisions, Using
the GHS PPO Network will mean Less out of pocket expense to you and your eligible
dependents.
We have listed below, the protocol that you will need to follow when taking
advantage of this cost saving benefit.
1. WHAT TO TELL YOUR PHYSICIAN
When your physician orders laboratory testing, that is for example - blood,
cultures, urinalysis testing - tell your physician that you have a laboratory
program. If you have your test(s) performed at one of your participating
laboratories, it is less or no Out of Pocket expense to you. Refer to the
information on your Health Care Plan Laboratory ID card.
2. IF THE PHYSICIAN OFFICE COLLECTS THE SPECIMEN
Ask the office to call a laboratory on your list in area for courier pickup. All
information regarding the GHS participating laboratories (Name, Address, Phone
Number, etc) is listed in this website. Follow the directions below and click on your
Local Union Health and Welfare Benefit Plan and you will be directed to the list of
participating facilities in your area.
3. WHAT DO YOU DO IF YOU GET A BILL FOR YOUR TEST(S) IN
THE MAIL
If you get a bill for your covered laboratory test(s) and you followed the program
protocol, DO NOT PAY IT!!!
Send the bill to your Plan Administrator. Wait until you receive an explanation of
benefits from your Plan Administrator. If there is a co-pay, you will be told what
amount is your responsibility and as a result of the deep discounts provided to your
benefit plan through GHS there may be none.
4. WHAT IS THE LABORATORY COST TO ME IF I DO NOT USE A
LABORATORY ON MY LIST?
If your Health and Welfare Benefit Plan has a co-pay, you will have to pay at that rate,
for example - your co-pay will be at 20%, 15%, or 10% of the billed amount. The Plan
is charged for the laboratory test at the usual customary and reasonable fee. Using the
GHS Laboratory program means that the charge for the laboratory test is discounted and
therefor your co-pay will be less out of pocket to you. At the same time, your Health and
Welfare Benefit Plan will be paying at a lower amount......saving both you and the
Plan! Truly a Win - Win for both.

"Allowed Amount". A specific Benefit Plan may have a different "discounted or allowed amount" from which your co-pay
will be applied. It is our experience that we are 20% to 30% less than any Benefit Plan "Allowed Amount" which will
result in a savings in your co-pay amount if GHS Program is followed. As a precaution we recommend you consult your
specific Benefit Plan booklet or Plan Administrator.
